This survey is to collect data on how cuts in Medicaid services are affecting people with disabilities of all ages. We are also collecting data on who in Medicaid is making the cuts

Part of the survey can be a little complicated to fill out. We are trying to find out when certain types of Medicaid services have been cut, and who at Medicaid is cutting them.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
